The rules of our referral program are easy to follow, making it simple to claim your rewards.
- The referring individual must be a current patient who has had an appointment within the past year and has no past due balance.
- Referrals must be new patients who are at least 18 years of age at the time of their first appointment.
- Referrals must list your name as the referral source on their new patient paperwork.
- Qualifying referrals must be made within the same year.
