Celebrating National Tooth Fairy Day 2020

Today is National Tooth Fairy Day! Kids can’t wait to lose their first tooth. It’s something big to look forward to – like their birthday. It’s such an exciting time because it’s something new and they know they will get some kind of “prize” with it. Whether it’s sparkly tooth fairy money, or a small toy or gold coin, kids are sure to love it. 

What Age Does a Child Lose Their First Tooth?

A child typically loses their first tooth around age 6. At Dental Reflections Dublin, we always add and subtract 6 months around this average because every child is different. The first tooth to be lost is one of the bottom front middle teeth followed by the one next to it. Once a child starts to lose teeth, they will lose the first 4 on the bottom front middle and then the top front middle. Then, they won’t lose teeth again for a few years – usually around age 10 or 11. 

Wiggly Teeth 

If your child’s tooth is lose, you can encourage them to wiggle it out. They can practice each day by wiggling it back and forth or pressing and holding it each direction for a few seconds. Many times, kids will shy away from brushing a loose tooth and the gums will get inflamed due to extra plaque in that area. Be sure to help them brush when they have a wiggly tooth or check over after. You do not have to press hard to remove plaque, just a gentle brush in this area will be sufficient. A mouthwash with fluoride is also good to use. Do not be alarmed if you notice teeth coming in on the back side of the wiggly teeth down in the bottom front. Dr. Rudi As-Sanie, a family dentist in Dublin, OH, says this is very common and does not affect anything. Once the wiggly teeth fall out the permanent teeth that came in behind will typically move forward into that space with everyday tongue pressure with eating and talking.

Frequency in Your Child’s Diet

We all know a toddler’s favorite food is not always the best for their teeth. It’s challenging enough to get a child to “eat the rainbow” with a balanced diet of all the food groups. Sometimes you have to bribe with a little bad to get a little good. Good and bad for the body don’t always translate to good and bad for the teeth. It’s also not about how much they consume when it comes to oral health, but how frequently they consume it.

Frequency is Key

If your child gets juice once a day, they should drink all of it at one time and with meals. Each sip of an acidic drink, (basically anything besides water) causes an acid attack in the mouth. This is what the bad bacteria feed off of and what causes a cavity. The mouth takes about 20 minutes to buffer those acids and by then, your kiddo is going for another drink. This means your mouth never has the chance to get to a healthy pH environment making the child at a very high risk all day for cavities. The same goes for snacking. If your child is eating pretzels for an hour+, the carbs break down into sugars leaving the mouth very acidic. Healthier between-meal snacks would be fresh vegetables like carrots, fruits like apples or strawberries, yogurt or cheese. These all have a more balanced pH and won’t leave the mouth so acidic.

Incorporate Water Into Your Child’s Diet

Water, water, water should be your child’s best friend. It truly is a natural “medicine” for kids and helps keep things functioning properly. Tap water contains fluoride, which helps prevent a cavity and a hydrated kid is less likely to get sick. Even flavored waters are a no-no, 0 calorie, and 0 sugars don’t mean they aren’t acidic. In fact, they are very acidic. Sticky foods like fruit snacks and even dried fruit like raisins are even more likely to stick into the teeth. A sugary treat is surely ok but the key is to limit it to meals and if the child is drinking plenty of water, their risk for decay will go down. Another huge factor is the nighttime brushing. Lots of parents report their child has a glass of chocolate milk every night before bed or a cup of milk. Milk contains natural sugars that settle into the grooves of the teeth and along the gumlines. When we sleep, our mouth gets very dry so we don’t have the saliva to help cleanse the teeth. Children’s Dental Health Month with Dental Reflections Dublin

February is one of our favorites – Children’s Dental Health Month. It’s the time to really focus on the dental health of children and how to keep their teeth clean and healthy. We know it can be a struggle to try to keep up with your child’s oral healthcare routine, but it truly sets the stage for their entire life. Starting good habits while the child is young will create a routine, the child will understand the importance of. This month, our blogs will center around different topics for a child’s dental health and hopefully offer many tips and tricks to keep your little one(s) teeth clean and cavity-free.

Tips for Brushing

Wording can be everything when you start giving your child options. Never ask them if they are ready to brush their teeth. What kid would ever be ready? Associate brushing with something else. Maybe it’s every night before reading books, or every night after bath time. Let them know at that time every night; now it’s time to brush. If you stay consistent, they know there is no option. Yes, they may put up a fight, but be adamant about it getting done, and they will get tired of fighting night after night. The same goes for mornings. Be sure you are getting that twice a day brush in. Lots of plaque bacteria form on our teeth at night when we sleep, and morning time is extremely important to brush to make sure food is less resistant to sticking to the teeth throughout the day. It is important to start using fluoride toothpaste at first tooth eruption. You only need a rice size amount smeared into the bristles of the toothbrush. Yes, we know your 6-month-old is not going to rinse and spit, and that’s ok. This meniscal amount of fluoride is not going to be toxic when your child swallows it. You can wipe out the remaining toothpaste with a washcloth using your finger after brushing.

Flossing and Mouthwash Use

Flossing is best at night and just once per day. We find it easier to floss with the string floss than the floss picks. You have more control when using the string. The picks tend to snap and may hit the gum, leaving the child more upset. Of course, it is whatever works best for you and your child. If you try flossing your child’s teeth while they are laying down at the end of a couch or standing in front of you facing forward with their head resting back on you, you can get a much better visual and floss easier. It all takes practice. In between, or flossing cavities are the most common among children since they tend to snack a lot and not floss as often as they should. Your child will need help with brushing and flossing until about the age of 8 years old. They can practice well before that, but following up or assisting is very important. Once your child is able to rinse and spit, a fluoride mouthwash is a great idea. This helps get into those small nooks and crannies. The toothbrush bristles even have a hard time reaching. The fluoride settles into these areas and helps strengthen, preventing cavities. Once a day, use for this is sufficient.

Be “Teeth Ready” for Back to School

Back to school is approaching fast and we want to give you the best tips to keep your child’s teeth healthy. First things first – get their checkup visit in before school starts! You want to be sure they don’t have any active decay or cavities in their mouth! According to the CDC, about 1 of 5 (20%) children aged 5 to 11 years have at least one untreated decayed tooth. The issue seen here is studies also show that children with active decay have issues with eating, speaking, playing and learning while at school. This is why preventative dental care is so important.

Check-up Visit

At your child’s checkup visit to the dentist, we can offer services that get them back to school ready. We assess their need for sealants to prevent cavities, apply a fluoride treatment to strengthen the enamel, and give tips on nutrition and brushing and flossing. We emphasize the importance of brushing twice a day for two minutes. We apply a disclosing solution to highlight missed areas while brushing so they know where to focus at home to effectively remove all the plaque. We also give them the tools they need if they are in braces to properly care for their teeth while at school which is a necessity.

Healthy School Lunches

This is a big one. Fill your child’s lunchbox with foods high in fiber, rich in fresh fruits and vegetables and even nuts and cheeses. These are all teeth-friendly and help promote a healthy pH in the mouth to prevent cavities. Avoid juices, sodas, sports drinks, dried fruits, fruit snacks and any other sticky, high sugar foods. Water and milk are best to drink. Pretzels and chips aren’t the best for the teeth, but other healthy choices listed above can offset and help remove the particles from the teeth better. They also dissolve more with saliva alone as compared with sticky, high sugar foods.

Vending Machines

This is another tough one. Vending machines are slowly improving but we aren’t there yet. Vending machine foods are filled with preservatives. Avoid candy bars, chips, sports drinks, energy drinks or soda. Sipping on anything but water throughout the day can significantly increase a child’s risk for decay. We see it all the time. Propel has water with electrolytes that is safe to drink with no added sugars or sweeteners. Other brands are coming out with their own versions as well.

Sports Mouthguard

Finally, it is important to protect your child’s teeth during any physical contact sport. This includes but is not limited to lacrosse, basketball, football, wrestling, and hockey. More and more schools are making this a requirement and make it a must that the guard be colored so the coach can see it in the mouth. A custom sports guard is well worth the money and protection, still allowing your child to talk and play as they normally would during a game vs. walking in our doors with a tooth in their hand. (Yes, we have seen it).

Benefits of Dental Implants

A dental implant serves the purpose of replacing a missing tooth. This restores chewing function where the open space is in the mouth. Not only can a dental implant fill a missing space, but there are also many other important benefits long term to choosing a dental implant over a partial, or just leaving the open space.

Drifting

When a tooth is missing the teeth around it and above or below it tends to shift. They naturally want the space to be filled and want to move into the open spot. The problem that occurs is this: Once the teeth start to move and drift, gum and bone problems begin to develop with the drifting teeth, leaving them compromised. The tooth above or below the space starts to supra-erupt meaning continue to come in because they have no opposing force. The tooth that is supra-erupting is also now compromised, leaving a poor prognosis for it as well. There is no way to tell how fast teeth around a missing space will start to move. For some, it could be years, others just months. Another downfall of waiting too long is that the open space may no longer be a candidate for an implant if the teeth around it have drifted too much.

Chewing Function

It is also important to restore chewing function from a missing tooth. Molars are the most common tooth in the mouth to be missing. These are the teeth we chew the most with, including hard, crunchy foods. When we are missing one or more molars, the smaller teeth in front of them begin to do more work, putting them at risk for fractures since they are not designed for this much force.

Implant Process

The first step to seeing if you are a good candidate for a dental implant is to schedule a consultation for an exam and radiographs of the area. Sometimes a bone graft is needed prior to implant placement to replace any missing bone in the area for optimum support. This is crucial for the success of the implant and is a very straightforward, minimally invasive procedure. Once the implant is placed, the area must heal for 4-6 months before the permanent crown is placed (this is essentially the tooth that will be in your mouth). Different Benefits of Fluoride

There are many benefits to incorporating fluoride into your daily oral health care routine. Fluoride is a mineral that occurs naturally in all water sources, including the oceans. Research has shown that fluoride not only reduces cavities in children and adults, but it also helps repair the early stages of tooth decay, even before the decay is visible. Fluoride is the best cavity fighter to help keep the whole family’s teeth strong — no matter their ages.* There are different types of fluoride treatments in and out of the dental setting that can be useful depending on a person’s decay rate.

Topical Fluoride

Topical fluoride comes in many forms, including toothpaste, mouthwash, and varnish. If a patient has a higher tooth decay rate, we may recommend a higher prescription strength fluoride toothpaste for them. An over the counter toothpaste has .24% sodium fluoride concentration. The prescription-strength has 1.1%, so 5x as much. Fluoride varnish has 5% sodium fluoride concentration and is applied during your 6-month checkup at your dental office. Consider this a larger dose to help strengthen the enamel and any weaker areas in the mouth. Children typically receive fluoride twice a year, and it is a great preventive for adults as well. For a patient that does not have a higher decay rate, a fluoride mouthwash like ACT or Listerine Total Care Zero is a great option to use twice a day after brushing.

Systemic Fluoride

This includes tap water with fluoride in it. Some other foods also contain naturally occurring fluoride like fruits and vegetables and even some seafood like shrimp and crab. Sometimes a fluoride supplement is recommended for kids in more rural areas without fluoridated water. Studies have shown those without fluoride in their tap water will have a higher risk of decay or developing cavities.

It is safe to use fluoride toothpaste at a child’s first tooth eruption. No need for the training toothpaste anymore. The American Academy of Pediatrics recommends a rice size amount smeared into the toothbrush which provides the perfect amount of fluoride and benefits even if the child is not yet spitting out the toothpaste and rinsing after brushing. Fluoride is a mineral we are very grateful for in dentistry as it helps in preventing cavities, which is our entire philosophy!

What To Expect at Your First Dental Visit to Dental Reflections Dublin

So, you scheduled your appointment as a new patient at our office. What’s next? We will send you a welcome email with instructions on how to complete your new patient paperwork on our website before you come in. This will save you time in our waiting room so we can get you back and get started. When we first get you seated, we will review your medical history you completed and address any concerns you may have. We will then take your blood pressure and begin the exam. At this time, you will be with one of our dental hygienists until the end of the appointment when you meet Dr. Rudi As-Sanie.

Initial Record Collecting

During the first exam, we are looking to see what dental work you have had done in the past, and charting it into our computer. This stage is our record collecting, so we have a thorough history of your dental history and what we see today. Next, we will update the measurements between your teeth and gums to determine the health of your gums. We will then take intraoral photos of the teeth with a digital camera wand, capturing details of each tooth. This sets a baseline so that if things were to change, we can refer back to how they looked previously. We can also use them to show you any concerns we may have that day.

Digital Radiographs

Next, we will take digital x-rays of your teeth to check for cavities. Looking in the mouth will only show us so much, and in order to do a comprehensive exam, we must take x-rays to see the health of the inside of the tooth.  With digital x-rays, radiation exposure is very minimal so no need to worry. Typically, we will take 4 bitewing x-rays of the back teeth to check for cavities, followed by a panoramic x-ray to show us the entire jaw and all of the roots of the teeth. This x-ray allows us to check for cysts in the bone or any other abnormalities that may show up. We may also take a 3D scan of your teeth if you have had a lot of dental work done in the past to be sure we are not missing anything.

Professional Cleaning

If your gum measurements were in a healthy range and we aren’t seeing any bone loss on the radiographs, we will then proceed with teeth cleaning. This includes removing hard and soft deposits from the teeth, bacteria below the gum line and polishing away any stains or plaque from the teeth. We then floss all of the teeth to help stimulate the gum tissue and remove any polishing agents from between the teeth.

Meet the Dentist/Final Exam

Towards the end of the appointment, the dentist will come in and do the final exam. This is reviewing all of the records we have collected and diagnosing or addressing any concerns she may have. A thorough review of the teeth is completed and she then will show you exactly what she is seeing in the mouth. This includes the x-rays, photos we took or any charting we completed. We recently had the pleasure of seeing one of our favorite bloggers, Shanisty Ireland, in our office. She is so inspiring, and her positive energy is contagious. As a mother of three, she is super transparent, which is one of the many things we love most about her. Her youngest boy had RSV which can be super scary for any mama. It can be life-threatening and stressful when a child is diagnosed with it. She is now an RSV advocate which is a big part of who she is. Shanisty even travels to tell others about her son’s experience and how he pulled through. Being a former news anchor, her infectious personality and public speaking skills are phenomenal.

What is Gingivitis?

Gingivitis. The dreaded word no one wants to hear pertaining to their mouth. Most toothpaste commercials mention it and say they can help prevent it. Any toothpaste can help prevent gingivitis if you actually brush your teeth with it. Gingivitis is the inflammation of the gums. This includes puffiness, bleeding, swelling, and redness. There are several causes and also ways to prevent it. The good news is that it’s reversible. Also – 47.2% of American adults have some form of it so don’t panic.

What Causes Gingivitis?

Gingivitis can be caused by not brushing or flossing. Plaque contains several different kinds of bacteria and when it is not removed, causes inflammation of the gums. Experts at Dental Reflections Dublin says you should brush twice a day for 2 minutes with a soft toothbrush to effectively remove the plaque that forms on your teeth throughout the day. Don’t forget to floss, it’s a key part of preventing gingivitis! Flossing cleans about 40% of the plaque off of the teeth that brushing can’t access. That’s almost half! Not flossing can also cause bad breath and cavities in between the teeth in addition to gingivitis. The good news is you only have to floss once a day.

Other Causes of Gingivitis

Systemic diseases or inflammation in the body can also cause gingivitis. Those with diabetes are much more likely to develop gum disease than those without it. This means they have to work that much harder to keep their blood sugar under control to keep their mouth healthy. Patients with uncontrolled high blood pressure can also cause gingivitis. The oral-systemic connection is a real thing and shows the importance of keeping both healthy.

Gingivitis Treatment

If your gums bleed when you brush or floss, don’t give up. The cleaner you keep your mouth, the more it should improve. Gingivitis only affects the gums which are why it is reversible. If it goes untreated for long enough, (not seeing a dentist regularly or doing your part at home) it can lead to periodontal disease. This causes bone loss around the teeth and is not reversible. It is much better to prevent gum disease than to manage it. There is no one cure for gingivitis; using a certain toothpaste or product. It’s mostly about technique and making sure you are removing the plaque effectively on a regular basis. Some other things you can add to your routine are an electric toothbrush and an antibacterial mouthwash. These can help remove plaque more effectively and reduce inflammation. Disclosing tablets are also a great tool to see exactly where the plaque is that you missed after brushing. They are available over the counter at most drug stores.
